You may download the necessary new client forms here. Please print these forms, fill them out, and then bring them with you to your first visit.
In addition to the new client forms, please fill out the form that corresponds to your challenge.
To download a form, right-click (ctrl-click for Mac) and choose Save As… from the menu. Choose where to save the file and click Save.
